CNA

 

I.  SUMMARY OF POSITION

  1. The CNA is a trained, certified, nonprofessional worker who assists professional nursing personnel in the care of the patient in the home in accordance with the standards, policies, and philosophy of the organization, the appropriate state practice acts, and federal regulations.
  2. Provides personal care, within scope of practice, on a per-visit or full-time basis.
  3. Works under the guidance and supervision of the staff registered nurse, the supervising nurse and the director of nurses.

II.  TASKS

  1. At the direction and under the supervision of the RN/PT/ST:
    1. Performs simple procedures within scope of practice
    2. Provides personal care.
  2. Attends staff meetings/case conferences and inservices.
  3. Participates in annual skills day.
  4. Observes for changes in health status and signs and symptoms, or deviations from normal status of patients.  Appropriately reports findings to supervising nurse or to patient’s primary nurse.
  5. Demonstrates courtesy, discretion, and compassion when dealing with patients, families, and staff members.
  6. Completes paperwork related to patient’s care in a timely and accurate completion of paperwork related to patient’s plan of care.
  7. Maintains the confidentiality of the patient as mandated by law, professional standards, and the company policies and procedures.
  8. Participates in performance improvement activities.
  9. Performs in the scope of practice after competency has been determined.
 

III.  QUALIFICATIONS

  1. Certified/licensed, as CNA in the state(s) in which he/she will work.
  2. Previous home health experience preferred.
  3. Working knowledge of simple procedures as extension of therapy services, personal care, ambulation and exercise, and household services essential to health care at home that follows company policy, federal state, and local regulations and is consistent with philosophy of company.
  4. Ability to communicate ideas effectively with others (written and oral).
  5. High School diploma or equivalent preferred.
  6. Ability to accept and utilize supervision as part of the home health care team.
  7. Knowledge of his/her scope of practice as dictated by Federal, State and local regulations.
  8. Respect for the need of confidentiality in regard to problems of the families being served by the home health agency.
  9. Personal qualities to deal with patients and their families.
  10. A strong commitment to the values, goals and philosophy of the delivery of home health services as set forth by the Board of Directors of the company.
